I just posted a text file, that describes what to do, when you run
into a problem such as yours. Please follow the instructions and let
us know, if it helped. One possibility is, that the 3c509b card maps
some constant number into part of the EPROM space; if this is indeed
the case, then the EPROM needs to be patched to take this fact into
account.

If I find some time (unlikely), I will write a Linux program that
scans for problems with ROM images. I can dual-boot one of the
machines in my office, but I can only do this after hours and I
usually work between 9am and 8pm...
