Thanks to all who have tested rom-o-matic.net and offered suggestions.

I have updated the site to use Etherboot 4.6.11, and have added the 
ability to select 4 ROM output formats:

  Floppy Bootable ROM Image      (.lzfdimg)
  Binary ROM Image               (.lzrom)
  LILO Bootable ROM Image        (.lzlilo)
  DOS .COM Executable ROM Image  (.com)

So now you can just generate a ROM image and do

  $ cat eb-4.6.11-3c509.lzfdimg > /dev/fd0

to make a test floppy.
