It looks like your tftp client is requesting a pretty large block size,
which might be alright, but your tftp server might not like it. I
suspect when you test it with another tftp client it requests a smaller
block size and works better.
Try making an Etherboot floppy from:
http://rom-o-matic.net/
Use version 4.7.24-mc1 and make your NIC type be "tulip".
When you press "Get Rom" you will get downloaded to your computer a file
called "tulip.lzdsk". You can then do:
cat tulip.lzdsk > /dev/fd0
under Linux or you can use RAWRITE under windows or DOS to make an
Etherboot floppy that should drive the card.
